Soccer jerseys!

We have created a logo and are offering soccer jerseys for those children/teens that want them and/or plan to participate in any games. Soccer shirts will be about $26 and include your child's last name on the back. Socks are included.

If you have family or friends that would like to make a donation to HAVEN SPORTS to support our uniform fund, please have them Paypal [email protected] with a note that it is for soccer jerseys. Any funds raised will reduce the cost of the shirts for all.

Shirts will take 2-3 weeks to come in, once ordered, so it is imperative that you add your information to this registration below. List your child's full name, including the last name they want displayed on their jersey AND general t-shirt size. (Example: youth x-small, youth small, youth medium, youth large, adult small, adut medium, adult large) Thanks!

We always need volunteers particularly with end of season stuff. Please consider helping to coach or assistant coach- we have amazing kids!

We particularly need one more parent for each of the following ages: 6-8, 9-12 and 12+. (We have head coaches for all of these ages- you would be assisting.)

I understand that HAVEN's soccer program is volunteer run. As a fellow parent, I will help where I am able and offer solution-seeking, respectful communication to the coaches, administrators, other parents, referees, children in the program, siblings, staff at the gym/field and anyone involved on an opposing team (if we play games). 

Note: During the first two practices, teen coaches will assess the players who are 12 to confirm they have the skills and strength to play at the teen level. If your child is asked to play with the 9-12 group after the first two practices, please know we are using an abundance of caution and care to ensure all players have the best opportunity and the safest experience. Thank you for your flexibility and understanding.

I agree to be on time and when we are unable to attend a practice (or game), I will communicate our absence prior to the practice (or game) day to my children(s)'s coaches via the team's preferred method of communication. 

My children and I agree to all Code of Conduct and behavioural expectations outlined by HAVEN homeschool collective in their Community Guidebook as well as, the Soccer Handbook and any expectations communicated by opposing coaches, referees, game/event hosts and venues. (These handbook's can be found on the website.) 

HAVEN Sports - Release of Liability Waiver

Please read the following:

___ I take responsibility for preparing my child(ren) with appropriate clothing depending on season, water bottles, appropriate protective and footgear.

___ I will disclose all of my child(ren)’s food allergies, sensitivities & physical restrictions.

___ I will disclose all of my child(ren)’s medications, medical conditions and/or restrictions where it might be appropriate/pertinent to mention.

___ I will disclose all my child(ren)’s behavioral considerations & emotional needs where it might be appropriate to mention.

___ I understand that HAVEN is a parent-run sports co-op, the fees are for equipment and remaining monies will be put towards next seasons’ sports. I am aware that HAVEN is family-centered and runs on the energy of parent & teen volunteers.

___ I understand the Assumption of Risk of Exposure to illness. By our family/my child engaging at/with HAVEN, I am aware that I agree to fully accept all known and unknown risks, including the potential risk of exposure to respiratory illnesses or other illnesses, diseases, or conditions.

___ Consent to First-Aid or Emergency Treatment. I consent to the application of first-aid or other medical or mental health services to be applied if needed in connection with an emergency health problem or potentially harmful situation during the participation, and I agree to hold HAVEN harmless as a result of any such injury or damage suffered due to the application of medical or mental health services or treatment. I also agree and consent that HAVEN may contact my Emergency Contact as shown on the bottom of this form and share detailed information about the emergency without violating my privacy rights.

As a HAVEN Sports participant or HAVEN Sports legal guardian, I have informed the HAVEN Sports leadership and/or coaches/volunteers of any medical needs I/my child(ren) may have and give HAVEN participants permission to administer basic first aid and prescription medicines as needed and directed. While I/my child(ren) am/is(are) participating in any HAVEN Sports activity, I take full responsibility for any physical injuries or conditions that I/my child(ren) may have at this time or have had in the past and I relinquish the rights to sue. HAVEN Sports, HAVEN Homeschool Collective, its volunteers, participating families and associated people will not be held liable for any injuries, illnesses, or damages that may occur during the course of this sports season while on the field or off field or any HAVEN Sports event or field trip or during transportation to and from practices, games, events, trips and/or locations.

I have carefully read this document and by signing below I consent to all parts of it. I understand that by signing the Release and Waiver, I voluntarily surrender certain legal rights.

I understand that NOT signing this waiver forfeits my family's and children(s)'s ability to participate in the HAVEN soccer program.
